We function as a patient advocate, ensuring each patient receives total care to maximize recovery potential.
We also have a preventive medicine program in place to keep an ongoing functional level of residence. This is a free service we
offer to nursing homes, retirement homes and assisted living facilities. We will determine a baseline for each patient and monitor
these levels on a quarterly basis.
We develop marketing strategies to enhance referrals to client facilities working with physicians, acute care facilities third
party payors and assisted living centers.
Working as a multi disciplinary team, Avalon Rehab customizes treatment plans for managed care patients to maximize patient outcomes
and reimbursement.
